GP Integration & Continued Care.
Support at the right time, without starting again.

Most traditional EAP models are designed for short-term support, often leaving employees to navigate ongoing care on their own once their allocated sessions end. For those who require continued support, this can mean starting the process again — finding a new practitioner, retelling their story, and managing long wait times. These barriers can be discouraging and frequently lead to employees disengaging from care before they are ready, impacting both individual wellbeing and workplace outcomes.

Connect My Mind is our continued care model, designed to remove this disruption and provide a seamless transition beyond EAP. Employees can continue working with the same best-matched practitioner, on the same platform, ensuring consistency, trust, and continuity of care.

Through our partnership with Hola Health, employees access integrated GP and allied health services directly within RISE, enabling connection to a Mental Health Care Plan where appropriate.
